You are requested to kindly grant certificate of Registration as required by section 15-B of the Jammu and Kashmir General Sales Tax Act, 1962 with effect from ..........................

I/we undertake to furnish security for an amount of Rs. 5,000/- in the form of cash deposit/Bank

Guarantee/Post Office or saving bank account pledged to the Sales Tax Commissioner. I/We also undertake to comply with the requirements of the provisions contained in section 15-A and 15-B of the said Act and the rules framed thereunder.
