Whereas you, a dealer have-

(a) failed to get yourself registered in accordance with the provisions of sub-section (1) of section 26 and whereas it is necessary to impose penalty under sub-section (1) of section 29 ;

(b) failed to deposit in accordance with the provisions of sub-section (2) of section 37 the amount collected by you in contravention of the provisions of sub-section (1) of the said section and whereas it is necessary to impose penalty under sub-section (4) of the said section ;

(c) contravened the provisions of section 63 by not issuing cash memorandum or bill against your sales and whereas it is necessary to impose penalty under sub-section (1) of section 64.

You are hereby directed to attend in person or by an agent at (place) .................. on (date) ............ at (time) ................ and to show cause why a penalty not exceeding Rs. ..............
