I/We hereby declare that I am/we are registered under the Kerala sales Tax Act, 1963, and possess registration certificate number......................and the foregoing statements and particulars are correct and complete to the best of my/ our knowledge and belief. Note :- 1. A separate application should be made for each different consignment for different destination.

2. In case of transport by road or water a copy of the permit should accompany the goods for production on demand.

3. The declaration/application by a registered dealer shall be singed by the proprietor of the business, if an individual, by the Karta or adult memder of an Undivided Hindu Family, a Marumakkathayam Tarwad, a family governed by the Keral Namboodiri Act, 1958 and an Aliya Santana family; by a partner in case of a firm ; by a managing director, secertary, or principal officer in the cse of a company; by a secertary, or principal officer or by a person manging the business or by a person duly uthorised to act on behalf in the case of a society, club, assocition, deprtment of Government or local authority.
